Waiver of Liability

By printing and signing this document, you acknowledge awareness that as a participant in group fitness classes you are participating in an activity that has inherent risks.

To make the class safer for you,

  1. When in doubt, move more slowly and with smaller range of motion than you typically do.
  2. Please be in charge of your own intensity level; all movements are suggestions, not requirements.
  3. If you have pre-existing injuries, please stay within your individual pain-free range of motion at all times.
  4. Take breaks as needed.

Please download the waiver, read in its entirety, then complete, sign, and date. Once complete you can bring the form to class or email to kiwadancefitness@gmail.com.
